Application Requirements
- Hong Kong residents aged 18 or above
- Certified illness as critical /irreversible by a HK registered medical doctor
- Wisher’s own wish
- No means test
- Belief / religion is not an approval factor

How to apply
Any individual who is either the wisher in person , next of kins, doctors, medical carers or social workers of wisher can send us an application or applying for a referral to us. Press the below buttons to know more or fill in the application form.
